Sometimes it just takes one nail to cause an issue if its driven in on a bit of an angle.
Ounce water finds alternative route other than down the roof to the gutters a capillary action occurs and draws the water into the defective nail area causing a leak.
Locating the area in question can be challenging on vaulted ceiling where there is no attic access.
This is where years of experience really comes into play.
